The writer should visit some of these O&G companies and see that they CONTINUALLY stress safety, policy, and procedures!

In the article, Fawcett states that "overconfidence masks warning signs -- we can see what we want to see rather than what's really there" sounds like a pretty common human trait regardless of whether we are overconfident, experts in the field, or the best at what we do. Pathological complacency sounds like another buzz word that is not needed.
